I certify that I am over the age of 18. If I am under 18 years of age, I confirm that I have obtained verifiable consent from a parent or legal guardian as required. I have voluntarily chosen to receive coaching from BESA Coaching (B.E.S.A LLC) and the nature and purpose of this service have been explained to me. I understand that coaching is intended to help me focus and work toward my personal and professional goals, which may include but are not limited to improved confidence, increased self-awareness, clarity of purpose, goal attainment, balance, and enhanced communication.
I acknowledge that coaching is a collaborative process. My Coach’s intention is to provide services and support based on the information I provide and the specifics of my situation. My Coach may offer recommendations to assist me, but I understand that it is my responsibility to choose whether or not to follow any recommendations. I recognize that results from coaching are not guaranteed and that outcomes depend on multiple factors such as my actions, mindset, habits, and circumstances.
I understand that if I feel the coaching relationship is not meeting my needs, it is my responsibility to communicate this to my Coach so that methods may be adjusted. Progress will be periodically tracked, and at the end of any committed period, my coaching outcomes and progress will be evaluated. Payment is due prior to each scheduled coaching appointment, and all payments are subject to the posted payment, refund, and cancellation policies as outlined at the time of purchase and in the Terms of Service.
I acknowledge that this agreement is for coaching and not for therapy or counseling. Coaching is not intended to diagnose, treat, or address medical or psychological conditions such as depression or anxiety, and these concerns should be addressed by a physician or licensed mental health professional. My Coach will not provide diagnosis or treatment for any medical or psychological conditions. I understand that coaching is not a substitute for counseling, psychotherapy, psychoanalysis, mental health care, substance abuse treatment, or any form of therapy. If I have or develop concerns regarding my mental health, I will seek the advice of a qualified professional.
I understand and agree that I am fully responsible for my well-being and for all choices and decisions I make during and after my coaching sessions. I understand that coaching is currently an unregulated industry and that my Coach is not licensed. All comments and ideas offered by my Coach are solely intended to support me in achieving my self-identified goals. I am providing my informed consent to participate in coaching and to allow my Coach to assist me in reaching my objectives.
I acknowledge that BESA Coaching and my Coach do not guarantee specific outcomes or success. Because coaching is not a medical service, I understand that insurance will not cover any costs related to coaching.
I understand that all information shared in coaching sessions will be treated as confidential by my Coach and BESA Coaching, except as required by law or as otherwise described in the Privacy Policy. I acknowledge that BESA Coaching collects, uses, and safeguards my personal data in accordance with its Privacy Policy, which includes details on session notes, communications, payment information, and my rights to access, correct, or request deletion of my information. I am responsible for providing accurate and current information as requested and understand that failure to do so may affect my access to coaching services.
By choosing BESA Coaching as my coaching provider, I affirm that I have carefully reviewed and comprehended this disclaimer and waiver. I confirm that I understand its contents. I agree to release my Coach and B.E.S.A LLC from any liability for claims, suits, actions, demands, or rights to compensation for damages that may arise from the advice I receive or any other aspect of my coaching relationship.
